Submit MMR Documentation

In compliance with New York State public health law, Columbia University requires all students to provide documentation of immunity to Measles, Mumps, and Rubella (MMR) before registering for classes in their first term of study.

Students must submit documentation of immunity to MMR to the Columbia Health Immunization Compliance Office prior to registering for classes. It is recommended that students complete this requirement at least 30 days prior to their scheduled registration date. For this reason, all incoming students are placed on a registration hold until the MMR documentation is received, verified, and processed. Delays in submitting documentation may result in delayed registration, fewer choices of classes, etc.
 
There are several ways to document immunity. Completed documentation may be submitted in the following ways:

We cannot accept immunization documentation via email for privacy and security reasons.

Please be sure to keep a copy of your fax or delivery confirmation as we cannot verify receipt of individual documents.

30 Days Before Registration

Submit proof of MMR immunity at least 30 days before class registration.*

Certify your meningitis vaccination decision online before class registration.

*If admitted less than 30 days before registration, your ability to register will be on hold until your MMR documentation is received and processed.

2 Weeks Before Your School Orientation

Submit a photo online for your University ID card. You will be able to pick up your card when you arrive on campus.
